Third Party Billing

We will bill third party carriers directly if complete and accurate information is provided on the test requisition form.

In addition to complete and accurate patient information, we must have the "Billing Information" section of the test requisition form completed as well as a copy of the patient's insurance card (front and back) or insurance face sheet attached to the test requisition form.

Patients are responsible for the payment of any co-payments, co-insurance, and deductibles or other allowed plan charges.

Direct Billing

We will bill a referring entity directly upon request in accordance with applicable government regulations.
• Payment terms are NET 30 days.
• Payments can be made by check or credit card
• Please refer to the NeoGenomics Laboratories invoice for additional information on payment.

Patient Billing/Responsibilities

As a service to patients, we will bill a patient's insurance carrier directly when we are provided with all of the necessary information.

If a patient is a Medicare beneficiary:

We will accept 100% of Medicare's approved amount for a test. The patient is responsible for the 20% balance of the Medicare approved amount.

If there is a deductible, the patient is responsible for the deductible.

For Medicare patients who have supplemental insurance, NeoGenomics Laboratories will bill the supplemental insurance for the difference between the approved amount and the actual Medicare payment plus any deductible.

NeoGenomics Laboratories will appeal any unpaid amount either for a unit short pay or for medical necessity denials.

If an advanced beneficiary notice is obtained, the patient will be responsible for and will be billed for the difference between the Medicare approved amount and the amount the Medicare carrier paid.

If a patient has a Third Party Insurance Carrier contracted with NeoGenomics Laboratories:

We will accept 100% of the contracted price for a test. We will bill according to the Explanation of Benefits (EOB) provided by the patient's insurance carrier.

If the health plan instructs us to bill for co-pays, co-insurance, deductibles, and non-covered services then we are required by law to adhere to these instructions.

If a patient has a Third Party Insurance Carrier not contracted with NeoGenomics Laboratories:

We will bill the patient's carrier. If the particular carrier does not pay 100% of our charge(s) for the testing, we will attempt an appeal to the carrier for payment of services.

If the third party insurance carrier pays the patient directly we will bill the patient to collect what the insurance company reimbursed the patient.
